For forms, which require the signature of a witness, the witness must be a third party. This witness can be your relative (but not your spouse or children), financial planner or customer service staff of Manulife.

Types of Request

Nomination/Revocation of Beneficiaries & Change of Trustee
Section 49L/49M (Insurance Act) (applicable for nominations made on or after 1 September 09)
Forms 1 to 3 (for irrevocable nominations)
Forms 4 to 6 (for revocable nominations)
Section 73 (Conveyancing and Law of Property Act) (applicable for nominations made before 1 September 09)

A trust is created under Section 73 Conveyancing and Law of Property Act when a policyholder takes a policy on his/her own life and names his/her spouse and./or child(ren) as beneficiaries. The beneficiaries cannot be revoked unless consent is obtained from the beneficiaries and the trustees

Surrender Application
Instead of surrendering your policy, you may wish to consider other alternatives as it may not be to your advantage to stop the coverage entirely.
For a traditional policy, alternatives such as a reduced paid up assurance or policy loan are available. For an investment linked policy, partial redemption of units could be effected. If you still wish to surrender your policy, please complete and provide the following:

Duplicate Policy
If you have lost / misplaced / destroyed your policy contract and require another copy, there will be an administrative charge of S$20 for this replacement. Please visit our office to complete a Notification of Lost Policy form.

Reinstatement of Policy
If you wish to reinstate your lapsed policy, please complete the Application for Reinstatement Form and mail to us together with your payment. If you are unsure of the amount of premium and interest required to reinstate your policy, please contact your Representative or call our Customer Service at 6833 8188.

Changes in Sum Assured / Change in Mode of Payment
If you wish to effect the above changes, please contact your Representative or call our Customer Service at 6833 8188.


Change of Personal Particulars
If you wish to change your personal particulars such as NRIC No or Name, etc., please complete the Application for change in Policy Form and mail back to us together with a photocopy of NRIC/Passport/Deed Poll.

Loan Application
You may apply for a loan if your policy has accumulated cash value. If you wish to have more information, please contact your Representative or call our Customer Service at 6833 8188.

Investment-Linked Top-Ups & Fund Switches
If you have an investment-linked policy, you may top-up the policy with a lump sum investment or switch units between funds.

Payment through Interbank GIRO
We encourage you to make your premium payments through GIRO. Please complete the Application Form for Interbank Giro and mail back to us.
We wish to inform you that the bank normally requires 2 month to process the new Interbank Giro application. In the meantime, please continue to send us your payment.
