eClaim submission

For mPal, mBreeze and ReadyProtect personal accident plans if the claimable amount is S$75 and below per accident, please submit the completed Personal Accident Claim form and relevant supporting documents through the "Personal Accident eClaim Submission" section below.

Original receipts are not required for claimable amount of S$75 and below per accident. If the total claimable amount exceeds S$75 per accident, original receipts must be submitted to us.

Please have the softcopy versions (max 2.5MB each) of the following documents as part of your submission:

Medical Reimbursement eClaims 
  • Policy owner’s completed Personal Accident Claim Form
  • Medical bill(s) 
  • Medical report(s) or doctor’s memo
Weekly Indemnity eClaims
  • Policy owner’s completed Personal Accident Claim Form
  • Medical certificate(s)
  • Hospitalisation leave (if applicable)
  • Medical record(s) or doctor’s memo

Please also ensure that you keep all original hardcopies of your medical bill(s), as we may request for them even after your eClaim has been approved.

Hardcopy submission

For medical reimbursement claims that are less than S$500 or weekly benefit claims not exceeding 30 days, we may waive the Attending Physician’s Statement if there is sufficient documentary evidence such as an “Inpatient Discharge Summary” or “Doctor’s Memo” showing the cause of the accident or injury. However, if deemed necessary, the Attending Physician’s Statement may still be required for claims that are less than S$500.
 
To help us process your claim as quickly as possible, please ensure that the required documents listed in the next section are completed and sent to us. We may need more time to process the claim if further clarification is needed, and will inform you if this occurs.
 
What you need to provide
  1. Personal Accident Claim Form – to be completed by the policyowner
  2. Attending Physician’s Statement – to be completed by the attending doctor
  3. Copy of the Insured’s or Claimant’s NRIC or passport
  4. Original tax invoice(s) / receipt(s)
  5. Copy of medical certificate(s)
  6. All relevant medical reports e.g. doctor’s memo, lab reports, A&E/inpatient discharge summary etc
  7. Copy of police report, if any
  8. Authorisation letter – required if a third party is assisting with the claim
FAQ
What is the estimated processing time?
• Acknowledgement of claim – via SMS, 2 working days after receiving the written claim notification or eClaim
• Assessment of claim – 10 working days after receiving all required documents
• Payment of cheque – 3 working days after the claim is approved

Who pays for the Attending Physician’s Statement or medical report?
The policyowner will have to bear the cost of the Attending Physician’s Statement or medical report, except for clarifications that Manulife requires.

Terms & Conditions – Applicable to Personal Accident claims submitted through Manulife Singapore’s website only
  1. Original receipts are not required for claimable amount of less than S$75 per accident. If the total claimable amount exceeds S$75 per accident, original receipts must be submitted to us.
  2. In spite of Point 1 above, we may request you to submit original receipts of your claim for the purpose of audit. If the original receipts cannot be produced for our verification, the paid claim may be treated as invalid and the amount paid will have to be returned to us.
  3. Our approval to a claim based on copy of receipt does not constitute to a waiver of our rights for requiring the original receipt to process any future claims.
