This section will guide you through the claims submission process for a Death Claim.
Before submitting your documents, please ensure that any documents in a foreign language are translated to English by an official Authority, Notary Public or Embassy.
Based on Section 61 (6) of the Insurance Act, the insurer may pay the first S$150,000 of the death proceeds to the ‘Proper Claimant’. The balance will be paid to the estate of the deceased upon receiving the Grant of Probate or Estate Duty Clearance.
All “Proper Claimants” including Controlling Person(s) of trusts will each have to complete and submit the Claimant Regulatory Tax Declaration Form.
If the policy owner is a company, additional documents will need to be submitted for the claim. The documents required are listed in the next section.
To help us process your claim as quickly as possible, please ensure that the required documents listed in the next section are completed and sent to us. We may need more time to process the claim if further clarification is needed, and will inform you if this occurs.
1. Death Claim Form – to be completed by the claimant
2. Claimant Regulatory Tax Declaration Form
3. Certified true copy of the death certificate – as verified by a Client Services Executive, lawyer or Notary Public)
4. Certified true copy of the claimant’s and deceased’s NRIC or passport
5. Proof of the claimant’s relationship to the deceased e.g. copy of Birth Certificate, Marriage Certificate etc.
6. Declaration of Beneficial Ownership – applicable for trust or keyman policies or if the Nominee is not a natural person e.g. an organisation or society, etc.
7. Attending Physician Statement (death) – to be completed by the last attending doctor for a death that took place overseas
8. Certified true copy of the deceased’s Will
9. Authorisation letter – required if a third party is assisting with the claim
For an accidental/unnatural death that occurred overseas, the following additional documents are required:
1. Certified true copy of burial/cremation documents
2. Copy of the police report or news report (if any)
3. Certified true copy of the post-mortem/toxicology report (if any)
4. Certified true copy of the coroner’s inquiry report (if any)

1. Corporate Owner Certification Form
2. Controlling Person Tax Residency Self-Certification Form (if applicable)
3. ACRA search report (within the past three months)
4. Signature of the person signing on behalf of the company, with the person’s name written clearly beside the signature
5. NRIC/passport of the person who is signing on behalf of the company
6. Corporate Policyowner Authorised Signatory Form (if the authorised signatory differs from our records)

Who pays for the Attending Physician Statement or medical report?
The policyowner will have to bear the cost of the Attending Physician Statement or medical report, except for clarifications that Manulife requires.
