Filing a Death Claim

We understand that this is a difficult time and we are here to support you.

What you need to know:

  1. If you are a Manulife customer, check that your mailing address, mobile number & email address has been updated with us. You may log in to our secure customer portal, MyManulife to update your contact details instantly. If your contact details are outdated, you will not be able to receive claim updates.
  2. All claimants, including Controlling Person(s) of trusts will each have to complete and submit the form below:

  3. For claims which require further information, you may be asked to contact the doctor to complete the Death Attending Physician Statement document (link below). The cost of obtaining this document will be borne by you. For such cases, we will keep you informed.

  4. Ensure you have these documents ready for upload, where applicable:
    • Death Certificate
    • Claimant Regulatory Tax Declaration Form (one per claimant)
    • Policy Owner's NRIC (front & back) or Passport
    • Insured's NRIC (front & back) or Passport
    • Claimants' NRIC(s), Birth Certificate(s), Passport(s)
    • Deceased Will, if any
    • Declaration of Beneficial Ownership (for Trust Keyman Policies or if nominee is not a natural person, e.g. organisation, society)
    • Proof of relationship with Deceased Life Insured (refer to Point 6)
    • Policy Owner's Bank statement or passbook with his/her name & account number if preferred payment is direct credit to a Singapore bank account
  5. For accidental/unnatural death, or for death occurred overseas, please provide the following additional documents:
    • Letter from Immigration and Checkpoint Authority (ICA). This letter is issued by ICA for Singaporeans or Permanent Residents who died overseas. It confirms receipt of the Singapore IC, passport and overseas death certificate
    • Attending Physician Statement by last attending doctor (for overseas death)
    • Repatriation Report (if body was repatriated to Singapore for cremation/burial)
    • Certified true copy of Burial/Cremation documents (for overseas death)
    • Copy of the Police Report/newspaper clipping (if any) if death was a result of accident/unnatural death
    • Certified true copy of the Post Mortem/Toxicology Report (if any)
    • Certified true copy of the Coroner's Inquiry Report (if any)
  6. The following proof of relationship documents will be required, where applicable:
    • Spouse - Marriage Certificate
    • Child - Birth Certificate
    • Parent - Deceased's Birth Certificate
    • Sibling - Deceased's Birth Certificate + Claimant's Birth Certificate
    • Nephew/Niece - Deceased's Birth Certificate + Claimant's Birth Certificate + Claimant's Parent's Birth Certificate
  7. For trust or keyman policies, or if the nominee is not a natural person (e.g. organisation or society), please complete the document below:

  8. If the policyowner is a company, please complete the forms below:        
    CORPORATE OWNER CERTIFICATION FORM                                                                                       
    CONTROLLING PERSON TAX RESIDENCY SELF-CERTIFICATION FORM
    Here is the full list of documents required for submission: 
    • Corporate Owner Certification Form
    • Controlling Person Tax Residency Self-Certification Form (if applicable)
    • ACRA search report (within the past 6 months)
    • Signature of the person signing on behalf of the company, with the person's name written clearly beside the signature
    • NRIC/passport of the person who is signing on behalf of the company 
    • Corporate Policyowner Authorised Signatory Form (if the authorised signatory differs from our records)
  9. All documents in foreign languages must be officially translated to English by a certified translator/interpreter.
