Instructions

Under Section 133 of the Insurance Act, you can make a revocable nomination and continue to retain full rights and ownership over the policy. This means that you can change or revoke a nomination at any time without requiring consent from the nominee(s). 


Only Policyowners who are 18 years old and above may make a revocable nomination. 

To make a revocable nomination, please complete the Revocable Nomination Form and mail it back to us with the necessary document(s) requested.

2. Copy of Policyowner’s NRIC
