Appoint a trustee or revoke the appointment of an existing trustee.

Instructions

Under Section 49L of the Insurance Act, you may appoint a trustee in addition to any trustees appointed previously, and/or revoke the appointment of a trustee, provided that there is at least one remaining trustee after the revocation.

To appoint or revoke a trustee, please complete the Appointment, or Revocation of Appointment, of Trustee of Policy Moneys Form and mail it back to us with the necessary document(s) requested.

FAQ

Who will need to sign the form to activate the change? 
For a Trust policy under S49L, the Policyowner and all Trustees who are not the Policyowner must sign the form.
