Appoint or revoke a trustee

This option allows you to appoint a trustee or revoke the appointment of an existing trustee.

What you need to know

Under Section 132 (formerly known as S49L) of the insurance Act:

A policyowner may appoint a trustee in addition to any trustees appointed previously, and/or revoke the appointment of an existing trustee, provided that there is at least one remaining trustee after the revocation.

Instructions

Download and complete the Appointment, or Revocation of Appointment, of Trustee of Policy Moneys Form.
Print the form and obtain signatures from the policy owner, all trustee(s), and two witnesses (signatories). Ensure all signatures are provided in wet blue ink.
Mail the completed original hardcopy form to us via the following address:

FAQ

Who will need to sign the form?

The policy owner, all trustee(s), and two witnesses (signatories) are required to sign the form. Ensure all signatures are provided in wet blue ink.

Who is eligible to act as a witness (signatory)?

All witnesses must be at least 21 years old.
A witness must not be the named beneficiary/nominee or the spouse of a beneficiary/nominee.

What is the estimated processing time for the request?

Requests received before 3pm will be processed in 3 business days.
Requests received after 3pm will be treated as received on the next business day.

If your request requires additional review or special handling, processing may take longer. We will contact you if further information or supporting documents are required and keep you updated on the status of your request.
