Nominate a beneficiary for a revocable nomination
A revocable nomination allows you to distribute the death benefits of your policy to your intended nominees (beneficiaries).
What you need to know
Under Section 133 of the Insurance Act:
- A policyowner can make a revocable nomination and continue to retain full rights and ownership of the policy.
- This means the policyowner can change or revoke a nomination at any time without requiring consent from the nominee(s).
Only policyowners who are 18 years old and above may make a revocable nomination.

Step‑by‑step guide
- Log in to your account via MyManulife or the Manulife SG App.
- Click on the “Services” tab, followed by "Nomination of beneficiaries“ and select "Make a revocable nomination (Form 4)".
- Ensure both the policyowner and witnesses (signatories) have Singpass access for electronic signing. Before proceeding with the nomination, review the information provided and confirm your personal information.
- Select the policies you want to nominate beneficiaries for. You can choose multiple policies.
- Enter the details of each nominee and indicate their respective share.
- Provide the details of your witnesses (signatories).
- Review and confirm your nomination details, then click “Next” to proceed to electronic signing.
- Click “Sign now” to be redirected to an external website for electronic signature.
- An access code will be sent to your registered mobile number. Enter this code to access the eSigning portal.
- Read and acknowledge the Declaration, then click “Next”.
- For each policy, click “Sign with Singpass”. You will need to scan the Singpass QR code provided on screen.
- Once signing is completed, click “Next” to return to MyManulife and submit your request.
- Select your preferred authentication method.
- Once submitted, your signatories will receive an email to complete their electronic signatures.
Apply for revocable nomination via hardcopy form submission
Who is this for:
- Policyowners who would like to nominate more than 8 nominees (beneficiaries).
- Policyowners who are unable to make an online nomination via our customer portal.
Instructions
- Download the Revocable Nomination Form and ensure all mandatory fields are filled out.
- Print out the form and obtain wet-ink signatures from both yourself and your two signatories (i.e.. witnesses).
- Mail the completed original hardcopy form to us via the following address:

Who will need to sign the nomination form?
The policyowner and two signatories (witnesses) must sign the form.
Who can be a witness for the nomination request?
- All witnesses must be at least 21 years old.
- A witness must not be the named beneficiary/nominee or the spouse of a beneficiary/nominee.
How long would it take to process the revocable nomination request?
The processing time for your request is 4 business days from the date we receive your submission for standard cases.
