Make a revocable nomination for your policy.
We have made it easier for you to complete the nomination form - you may complete the PDF form directly on your device (e.g. computer) before printing it out for signing by yourself and your two witnesses. Completing it digitally ensures mandatory fields are not omitted that may delay your nomination process.
Under Section 49M of the Insurance Act, you can make a revocable nomination and continue to retain full rights and ownership over the policy. This means that you can change or revoke a nomination at any time without requiring consent from the nominee(s).
Only Policyowners who are 18 years old and above may make a revocable nomination.
To make a revocable nomination, please complete the Revocable Nomination Form and mail it back to us with the necessary document(s) requested.

Who will need to sign the nomination form?
For non-trust policies, the Assignee or Policyowner must sign the form.
Make a trust nomination to transfer all policy ownership rights to a beneficiary.
Apply to revoke the trust nomination for your policy.
Appoint a trustee or revoke the appointment of an existing trustee.
Revoke an existing revocable nomination and make a new nomination.
Inform Manulife of the revocation of a revocable nomination via a will / assignment.
