Remove a beneficiary from a trust nomination
If you wish to remove a beneficiary from your trust nomination, you will need to revoke your existing trust nomination and submit a new trust nomination reflecting your updated list of beneficiaries.
Before you begin
Please ensure that:
- All beneficiaries aged 18 and above have provided their consent to revoke the trust nomination.
- If any beneficiary is below 18 years old, written consent must be obtained from:
- their parent or legal guardian (who must not be the policy owner), or
- a trustee (who must not be the policy owner).
How to submit your form
- Download and complete the Revocation of Trust Nomination Form.
- Print the form and obtain signatures from the policy owner, all trustee(s), and two witnesses (signatories). Ensure all signatures are provided in wet blue ink.
- Mail the completed original hardcopy form to us via the following address:

Frequently Asked Questions
Can I remove only some of my beneficiaries from a trust nomination?
No. Beneficiaries cannot be removed individually from an existing trust nomination.
If you wish to change the list of beneficiaries, you must first revoke your existing trust nomination and then submit a new trust nomination with your updated beneficiary details.

Who will need to sign the form?
The policy owner, all trustee(s), and two witnesses (signatories) are required to sign the form. Ensure all signatures are provided in wet blue ink.
Who is eligible to act as a witness (signatory)?
- All witnesses must be at least 21 years old.
- A witness must not be the named beneficiary/nominee or the spouse of a beneficiary/nominee.
What is the estimated processing time for the request?
- Requests received before 3pm will be processed in 3 business days.
- Requests received after 3pm will be treated as received on the next business day.
If your request requires additional review or special handling, processing may take longer. We will contact you if further information or supporting documents are required and keep you updated on the status of your request.
